Noticeboard

The Surgery will open as normal on Friday 8th May 2020. Medication can be collected between 10am -12pm and 3pm - 5pm. 

We are sad to announce that Dr David Woodhead will be leaving us at the end of April due to a re-location with his family. He will be missed by staff and patients alike and we wish him all the best for the future.

If you have a prescription or medication to collect the Surgery will ONLY be open between 10am-12pm and 3pm and 5pm.  A member of staff will be at the side window in the porch area to assist you.

New Patient Registration

If you live within our practice area and wish to register with us, simply come to the surgery (bringing your medical card if you have one). You will be given a registration form and new patient questionnaire to complete or you can download these forms and hand them in to reception. 

Please complete the registration form below and one of the patient questionnaires below depending on your age group. Without two forms for each patient we will be unable to complete your registration.

Changes to Personal Details

It is very important that you notify us immediately of any changes of name, address or telephone number and email address. There are many reasons why we may need to contact you.
