The Surgery will be closed from 1.00pm on Thursday 19th March for staff training and will not reopen until Friday 20th March. If you need to speak to a doctor urgently, please telephone the out of hours service on 111. We apologise for an inconvenience caused.  

Advice to travellers returning from Wuhan, China

If you have travelled to Wuhan City in China in the last 2 weeks and feel unwell, call NHS 111 for advice.

Do not go straight to a doctor's surgery or hospital


We now have an appointment checking in touch screen in the waiting area of reception. This will save patients having to wait at reception to check in.

If you need assistance to be shown how to use it please ask a member of staff. 


Missed Appointments -If you are unable to attend your appointment please inform the surgery so we are able to offer these to other patients.  In January there were 59 Doctor and 61 Nurse appointments that were unattended.

Prescription forms (ie not medication) are now collected from the Enquiries Desk.  Medication is collected from the Dispensary.

Registration for On-Line Access

To register for these services, please complete the appropriate form below or collect a form from the surgery.  You will need access to your own personal email address to receive these services.

Two forms of ID will be required when attending the Surgery.  One photo ID and one Address ID (not utility bills).  You must attend the Surgery in person so that your identity can be verified by a member of staff.

Over 16's Patient Access Registration Form

For patients who have not yet reached their 16th birthday access can be given through a parent or guardian's email account.  Access will be for appointments and repeat medications only.  ID will still be required if available.

When a patient reaches 16 years of age a new account must be applied for (see application form above for 16+).
